Roles & Responsibilities

• Provide skilled physical therapy care in patients' homes across an assigned geographic region
• Complete accurate OASIS assessments and timely documentation in Cerner EMR
• Develop and implement individualized treatment plans for ortho, stroke, joint and spinal injury patients
• Collaborate with OT, Speech, Rehab Aides, PTA/OTA and interdisciplinary teams to maximize patient outcomes
• Communicate effectively with patients, families and caregivers to educate and promote safe home recovery
• Participate in orientation and unit training (1.5–2 weeks) to learn local processes and expectations
• Cover scheduled weekend rotation (every 3rd weekend) and provide holiday coverage when necessary

Qualifications

• Current, unrestricted Physical Therapist license (able to submit in CA)
• Minimum 2 years of clinical experience (home health or related settings preferred — at least 1 year PT/OT/Speech experience is requested)
• BLS certification required
• Experience with OASIS documentation (required)
• Familiarity with Cerner or similar EMR systems preferred
• Comfort working independently in home settings and traveling within an assigned region
• CCC-SLP or CCC-A credentials are a plus for Speech positions; PTA/OTA support roles accepted

Education

• Graduation from an accredited Physical Therapy program (DPT or equivalent) and current PT licensure appropriate for practice

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Proficiency with OASIS assessments and home health regulatory requirements
• Skilled charting and documentation in Cerner or equivalent EMR
• Experience treating orthopedic, stroke, joint and spinal injury patients
• Excellent communication, time management and patient education abilities
• Ability to collaborate with interdisciplinary teams and supervise rehab aides/assistants
