Roles & Responsibilities

• Perform comprehensive initial assessments of clients and families to identify home health needs
• Develop, initiate, and revise individualized plans of care based on assessments and changing needs
• Provide medication administration and treatments as prescribed by physicians
• Document clinical findings and progress notes accurately in HomeCare HomeBase and communicate with providers
• Coordinate discharge planning and ensure necessary equipment and services are in place
• Act as case manager when assigned, coordinating care across the patient’s team
• Instruct, supervise, and evaluate home health aide care to ensure quality delivery
• Maintain regular physician communication and obtain/implement physician orders as required

Qualifications

• Active RN license (required)
• BLS certification (required)
• Minimum 1 year of Home Health and/or Hospice nursing experience
• Valid driver’s license and current auto insurance (required)
• Experience with HomeCare HomeBase EMR preferred
• Ability to provide care across a roughly 50-60 mile service radius and participate in on-call rotation
• Willingness to provide holiday coverage and follow agency RTO/submission rules
• No full-time/part-time/PRN employment with CommonSpirit (including CHI or Dignity) within the past year as specified

Education

• Active RN licensure required
• BSN not required

• Strong patient assessment and clinical judgment skills
• Care planning and nursing diagnosis formulation
• Medication administration and treatment competency
• Accurate clinical documentation and physician communication
• Case management and coordination skills
• Comfort working with diverse populations: adult, geriatric, pediatric
• Time management, organization, and ability to manage a caseload
• Professional bedside manner and family-centered teaching skills
