Shift Type: Rotating 12-hour shifts (evening/night) — typical windows include 12:45 PM - 1:15 AM and 2:45 PM - 3:15 AM; schedule may vary across any day of the week; every-other-weekend required; holiday coverage as needed (Guaranteed 36 hours/week)

Roles & Responsibilities
- Facilitate timely transfers by coordinating acceptance with accepting physicians and specialist teams
- Perform clinical assessments and document vitals and patient reports for transfer cases
- Request and manage bed placements using the TeleTracking system
- Use Cerner or Epic to chart clinical information and maintain accurate transfer documentation
- Communicate clearly and promptly with physicians and specialists via TigerConnect/text and other channels
- Prioritize and manage multiple concurrent cases (typically 10–15 at a time) while maintaining clinical judgment
- If functioning as an LVN, manage refer-outs when internal placement within the system is not available
- Collaborate with supervisors and the leadership team to ensure safe, efficient patient flow
Qualifications
- Active RN or LVN license (California) required
- Minimum 2 years' experience in ICU, ED, or Transfer Center (job listing notes 3 years preferred in some fields)
- BLS certification required
- Clinical assessment skills and strong critical care judgment — able to follow algorithms
- Experience with Cerner or Epic EMR required; TeleTracking and TigerConnect experience highly preferred
- Proven ability to multitask, manage many simultaneous cases, and provide excellent customer service
- Must complete the Relias Dys exam through UNA prior to acceptance (pass/fail; two attempts allowed; screenshot of attempts required). Relias Dys within last 12 months may be accepted
- Two supervisory references required at submittal (one within 12 months and one within 3 years)
- Candidates cannot have been directly employed by CommonSpirit/CHI/Dignity in the past year (PRN through outside firms is acceptable)
Education
- Active RN or LVN license (state of California)
- Associate degree or diploma in Nursing required; BSN preferred but not required
- BLS certification required
Knowledge & Skills
- Transfer coordination and bed placement workflows
- Clinical assessment, vitals interpretation, and critical care triage
- EMR charting in Cerner or Epic
- Bed management using TeleTracking
- Secure clinical communication via TigerConnect/text
- Comfort with Google Apps and managing multiple chat streams
- Strong multitasking, organization, and customer service skills
- Ability to learn quickly and follow department algorithms and protocols
