Shift Type: Days, 8-hour shifts (exact start/end times vary by unit); typical schedule Monday-Friday with every 3rd weekend and first-to-float assignment. 12-hour night option available for candidates with ED background who are willing to work nights.

Roles & Responsibilities
- Coordinate acute care case management and utilization review to ensure timely, safe patient transitions
- Lead discharge planning activities and collaborate with multidisciplinary teams to remove barriers to discharge
- Perform accurate charting in Cerner and maintain thorough documentation for utilization review
- Manage patient assignments (approximate ratio 1:20) while prioritizing patient safety and outcomes
- Communicate with physicians, social work, and community resources to support continuity of care
- Float across adult areas of the hospital as needed and cover responsibilities based on clinical competency
- Maintain excellent attendance and professional presence; take required breaks and lunches
- Serve as a patient advocate, ensuring needs are met and care plans are followed through
Qualifications
- Active Registered Nurse license required (California)
- BLS certification required
- Minimum 2 years of RN experience (posting); note the unit requests minimum of 3 years hospital experience and acute level care coordination experience
- MUST have prior California Dignity/CommonSpirit acute care experience to be considered
- Proficient with Midas, InterCall, Cerner, Curaspan, and Microsoft Office
- Experience in discharge planning and utilization review
- Able to provide 2 supervisory references at submittal (one within 12 months, one within 3 years)
- Willingness to float to all adult areas; women's health coverage not expected if candidate lacks that background
- ED background required for the 12-hour night option
Education
- Active RN license in California
- BSN not required
Knowledge & Skills
- Strong knowledge of case management and utilization review processes
- Charting proficiency in Cerner and familiarity with Curaspan
- Comfortable using Midas and InterCall for clinical workflows
- Skilled in discharge planning, care coordination, and acute care patient management
- Ability to multitask in a fast-paced acute care environment and adapt quickly
- Excellent communication and teamwork skills across disciplines
- Basic computer literacy including Microsoft Office
