Roles & Responsibilities

• Perform comprehensive initial nursing assessments to determine discharge needs
• Develop and coordinate discharge plans including home health, DME, nursing homes and appointments
• Work collaboratively with multidisciplinary teams to ensure safe, timely patient transitions
• Manage case load of ~15-28 patients per day with prioritization and clear documentation
• Communicate with payers and complete prior authorizations as required
• Provide patient advocacy for uninsured and homeless populations to secure appropriate resources
• Document all care coordination and discharge activities accurately in Cerner
• Participate in utilization review activities as needed to support efficient care delivery

Qualifications

• Active RN license eligible for practice in California
• Minimum 3 years nursing experience in an acute hospital setting
• Prior care coordination/discharge planning experience in a CommonSpirit/Dignity/CHI hospital (must-have)
• Current BLS certification required
• Experience charting in Cerner (electronic medical record proficiency)
• Strong customer service skills with excellent interpersonal and written communication
• Ability to multi-task, work independently, and adapt in a fast-paced environment

Education

• Associate Degree in Nursing (ADN) or Nursing diploma required; BSN preferred but not required

Knowledge & Skills

• Thorough knowledge of discharge planning processes and community resources
• Familiarity with utilization review and case management workflows
• Proven ability to work with multiple payers and manage prior authorizations
• Competence in care coordination for complex social needs (uninsured, homeless)
• Strong documentation skills within Cerner and accurate charting habits
• Excellent time management and patient prioritization abilities
