Roles & Responsibilities
- Evaluate patients’ mobility, function and pain to create individualized treatment plans
- Deliver evidence-based therapeutic interventions to restore movement and improve quality of life
- Collaborate with physicians, nurses and rehab team members to coordinate care and goals
- Document progress clearly and timely in the medical record to support continuity of care
- Educate patients and families on home programs, safety strategies and self-management
- Monitor and adjust treatment plans based on patient response and established outcomes
Qualifications
- Current, active Physical Therapist license in the state of practice (or eligible to obtain)
- Degree in Physical Therapy (DPT, MPT or equivalent) from an accredited program
- Basic Life Support (BLS/CPR) certification or willingness to obtain
- Clinical experience in outpatient or inpatient rehab settings preferred
- Strong communication and teamwork skills; comfortable collaborating with a multidisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred; or Master's/Bachelor's in Physical Therapy with appropriate licensure
Knowledge & Skills
- Skilled in musculoskeletal and neuromuscular assessment and treatment
- Proficient with therapeutic exercise, manual therapy and functional mobility training
- Comfortable with patient education, fall prevention and assistive device training
- Familiarity with common rehab documentation practices and EMR systems
- Strong clinical reasoning and outcome-driven treatment planning
