Roles & Responsibilities

• Coordinate patient care and discharge planning to ensure safe, timely transitions
• Conduct comprehensive case management assessments and develop individualized plans
• Collaborate with physicians, social workers, and multidisciplinary teams to optimize patient outcomes
• Document care and case decisions accurately and efficiently using Midas and Meditech
• Advocate for patients to secure appropriate services and community resources
• Review utilization and manage authorizations to support appropriate care delivery
• Communicate clearly with patients and families, providing education and support throughout the care journey

Qualifications

• Current Registered Nurse (RN) licensure
• Minimum 2 years of professional experience (as listed)
• Prior experience in case management at an HCA facility (required)
• Proficiency with Meditech and Midas documentation systems (required)
• Strong documentation skills and attention to detail
• Excellent communication and care coordination abilities

Knowledge & Skills

• Thorough knowledge of case management practices and discharge planning
• Familiarity with Meditech and Midas documentation workflows
• Skilled in interdisciplinary collaboration and care coordination
• Ability to manage a high patient ratio while maintaining quality documentation
• Time management, critical thinking, and patient advocacy skills
