Shift Type: Monday-Friday, 8:30 AM - 5:00 PM (Day) or alternate Sunday-Thursday, 8:30 AM - 5:00 PM (Day) — 40 hours/week guaranteed

Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Manage a hospice caseload of up to 15 patients with compassion and clinical excellence
- Conduct 4–5 home visits per day, delivering routine, GIP, continuous, and respite nursing care
- Perform admissions, SIA (stabilization in assessment), and death visits professionally and sensitively
- Complete same-day charting and documentation in WellSky Consolo to ensure continuity of care
- Coordinate care with the interdisciplinary team and community resources to support patients and families
- Maintain required screenings, vaccinations, and compliance documentation
- Travel within a 60-mile service radius using a reliable vehicle and maintain valid auto insurance
Qualifications
- Active Washington RN license or Compact license (required)
- One or more years of current Hospice Case Management experience (required)
- BLS (AHA) certification (required)
- Valid driver’s license, auto insurance, and reliable vehicle to cover the service area
- Clean background checks in WA and OR and drug screening clearance
- Influenza, TB, and COVID vaccination records as required
- Two supervisory references from the last two years (required)
- Acute/Med-Surg experience highly preferred
- Oregon RN license encouraged if able to float to OR sites; application for OR licensure required upon contract acceptance
Knowledge & Skills
- Strong hospice case management and home-visit nursing skills
- Comfort providing end-of-life care including admissions and death visits
- Excellent charting and documentation habits; same-day documentation expected
- Ability to manage time and travel logistics across a large service area
- Compassionate communication with patients, families, and interdisciplinary teams
- Familiarity with WellSky Consolo EMR (or willingness to learn quickly)
- Clinical competence in routine, GIP, continuous, and respite nursing services
