Shift Type: Day shift, 8:00 AM - 5:00 PM, rotating weekends (every other)

Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Coordinate and lead bedside case management to ensure safe, timely patient transitions
- Develop and execute discharge plans in collaboration with patients, families, and the care team
- Document thoroughly and accurately in Meditech/Midas to keep the care continuum moving
- Serve as a key communicator with physicians, social workers, and post-acute partners
- Manage throughput and patient flow to optimize unit performance and patient experience
- Have compassionate but clear conversations with patients and families about plans of care
- Advocate for patients’ post-acute needs and ensure appropriate placement and resources
Qualifications
- Current RN (Registered Nurse) credential—ready to practice in the assignment location
- Minimum 1 year of bedside case management experience (required)
- Experience with Meditech or HCA (must-have per role)
- Knowledge of post-acute care settings and discharge planning
- Comfortable with rotating weekend coverage (every other weekend)
- Strong communication skills and the ability to lead difficult conversations with empathy
Knowledge & Skills
- Proficient with EMR systems, specifically Meditech and Midas
- Expertise in discharge planning and coordination of post-acute services
- Throughput management to keep patients moving safely and efficiently
- Bedside case management skills with patient-centered focus
- Collaborative team skills—work well with physicians, social workers, and ancillary staff
- Strong documentation practices and clinical decision-making
