Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment to post-acute patients with compassion and clinical excellence
- Develop individualized, measurable care plans and revise them as patients progress
- Use evidence based interventions and therapeutic modalities to maximize function and safety
- Document patient progress clearly and timely in the facility EMR to support continuity of care
- Collaborate with nurses, occupational therapy, speech therapy and case managers to achieve patient goals
- Educate patients and families on home exercise programs and safe discharge planning
Qualifications
- Current California Physical Therapist license required
- Doctor of Physical Therapy or Master of Physical Therapy preferred
- Basic Life Support CPR certification required or obtained upon hire
- Experience in post acute, outpatient or rehab settings preferred
- Strong documentation skills and comfort with electronic medical records
- Reliable, professional, and able to work well within an interdisciplinary team
Education
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy
Knowledge & Skills
- Patient assessment and rehabilitation planning
- Therapeutic exercise and manual therapy techniques
- Fall prevention and safe mobility training
- Skilled use of therapeutic modalities and adaptive equipment
- Effective communication with patients, families and clinical teams
- Clinical documentation and outcomes tracking
