Roles & Responsibilities

• Evaluate patients to create individualized, goal-oriented treatment plans
• Deliver skilled physical therapy interventions to improve mobility and function
• Document patient progress accurately and timely in the facility record
• Collaborate with nursing, therapy colleagues, and families to ensure cohesive care
• Educate patients and caregivers on safe mobility, home programs, and fall prevention
• Monitor and modify treatment plans based on patient response and clinical judgment
• Maintain compliance with facility policies and regulatory standards

Qualifications

• PT or DPT credential (primary skills listed as PT / DPT)
• Any experience level accepted — new graduates encouraged to apply
• Ability to obtain or hold state PT licensure (confirm Ohio requirements with recruiter)
• Strong communication skills and a team-oriented mindset
• Current BLS/CPR certification as required by facility or employer (verify with recruiter)

Education

• DPT or PT degree preferred; open to new graduates as noted in the job posting

Knowledge & Skills

• Knowledge of physical therapy evaluation and treatment planning
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Skilled in safe patient handling, transfers, and mobility training
• Competent documentation practices and clinical reasoning
• Ability to collaborate with interdisciplinary teams and engage families
• Patient education and fall prevention strategies
