Shift Type: Rotating 4x10 night shifts, 7:00 PM - 5:30 AM, 4 shifts per week (includes every other weekend); night shift
Hours per Week: 60
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Conduct thorough psychosocial and biopsychosocial assessments to understand patient needs
- Develop person centered discharge plans and coordinate safe transitions of care
- Perform safety planning and screen for suicidality, intervening when necessary
- Provide brief clinical interventions and crisis support in the emergency setting
- Navigate community resources and refer patients to appropriate services
- Collaborate closely with RN case managers and the interdisciplinary team
- Document assessments, interventions, and discharge plans accurately in Epic
- Complete required reports of harm in accordance with state statutes
- Support pediatric and adult patients with age appropriate approaches and referrals
Qualifications
- LMSW or LCSW required; pending Alaska license is accepted
- Master of Social Work from a CSWE accredited program preferred
- Minimum 1 year experience required; acute care experience is mandatory
- Emergency Room experience preferred and ER care management experience a plus
- Comfortable working nights and able to cover every other weekend
- Ability to float within scope to meet facility needs
- Proficient with Epic or other electronic charting systems
- Familiarity with state reporting requirements and protective services
- Strong communication, documentation, and care coordination skills
Education
- Master of Social Work (MSW) from a CSWE accredited program
Knowledge & Skills
- Psychosocial and biopsychosocial assessment
- Discharge planning and care coordination
- Safety planning and suicidality screening
- Brief clinical interventions and crisis intervention
- Resource navigation including Medicaid, Medicare, housing and community programs
- Experience with Epic charting and clinical documentation
- Geriatric Depression Scale (GDS) and Mini Mental State Examination (MMSE) familiarity
- NASW Code of Ethics and culturally competent practice
- Collaborative work with interdisciplinary teams and RN case managers
- Knowledge of major disease processes and social determinants of health
