Roles & Responsibilities

• Provide compassionate direct nursing care to residents at Hopkins Center, putting patient dignity first
• Administer medications and treatments safely and accurately according to facility policy
• Perform timely assessments and monitor patient condition, reporting changes to the care team
• Collaborate with RNs, physicians and interdisciplinary staff to deliver coordinated care
• Document care thoroughly and promptly in the electronic medical record
• Educate patients and families about care plans, treatments and discharge instructions
• Follow infection control, safety and compliance standards to protect patients and staff
• Support admissions, transfers and discharges to ensure smooth transitions of care

Qualifications

• Valid Licensed Practical Nurse (LPN) license (able to practice in Pennsylvania)
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable with 12+ hour day shifts and collaborative clinical environments
• Strong communication and interpersonal skills with a patient-centered approach
• Ability to pass background check and drug screening as required by the facility

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate charting
• Basic nursing assessments and vital sign monitoring
• Wound care and basic dressing changes
• Familiarity with infection control and safe patient handling
• Experience with electronic medical records and documentation best practices
• Time management, prioritization and teamwork in a fast-paced setting
