Roles & Responsibilities

• Provide compassionate direct patient care and support daily living needs
• Administer medications and treatments per physician orders and facility policy
• Monitor and report patient condition changes promptly to the care team
• Document patient care accurately in the electronic medical record
• Collaborate with RNs, physicians, and interdisciplinary team members to coordinate care
• Assist with admissions, transfers, and discharge planning as needed
• Promote a safe, clean environment and adhere to infection prevention protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license; Pennsylvania licensure preferred
• Current BLS (Basic Life Support) certification or willingness to obtain
• Completion of an accredited practical nursing or vocational nursing program
• Ability to pass background check, drug screen, and health screenings
• Previous experience in long-term care, rehab, or acute care preferred but new grads with strong clinical skills are welcome

Education

• Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

• Solid skills in medication administration and basic nursing procedures
• Comfortable with patient assessments and documenting findings clearly
• Good communication and teamwork skills to collaborate with clinical staff
• Familiarity with electronic medical records and charting best practices
• Time management and prioritization in a fast-paced care setting
• Basic wound care and infection control knowledge
