Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Hopkins Center
• Administer medications safely and document care accurately
• Monitor patient status and communicate changes to the care team promptly
• Assist with activities of daily living and support resident dignity and comfort
• Collaborate with interdisciplinary team members to deliver person-centered care
• Support admissions, discharges, and care transitions with professionalism

Qualifications

• Active LPN license in Pennsylvania required
• Current BLS/CPR certification (or obtain upon hire) recommended
• Comfortable with medication administration, documentation, and basic assessments
• Professional, patient-centered approach and strong communication skills
• Travel/contract experience a plus but not required

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Practical nursing skills including medication administration and wound care
• Ability to observe, assess, and report changes in patient condition
• Familiarity with electronic charting and accurate clinical documentation
• Strong interpersonal skills for working with residents, families, and staff
• Time management and ability to prioritize in a busy care setting
