Roles & Responsibilities
- Evaluate patients across the lifespan and develop realistic, measurable short-term and long-term treatment goals in collaboration with patients, families, and the care team
- Provide and assess the effectiveness of therapeutic treatments tailored to each patient’s age, ability, and goals
- Document care accurately, legibly, and in a timely manner following department policies
- Educate patients, family members, and staff to support goal attainment and safe discharge
- Communicate clearly with referring providers, case managers, and interdisciplinary team members about patient status and discharge planning
- Delegate appropriate treatments to therapy assistants and provide supervision and direction
- Complete reevaluations as indicated and ensure continuity of care with therapy assistants
- Complete daily billing and work with reception to schedule patients efficiently
- Participate in professional development, student clinical instruction, and quality improvement initiatives
- Adhere to hospital safety policies and contribute positively to department culture and mission
Qualifications
- Bachelor's Degree in Physical Therapy from an accredited program - Required
- Licensed physical therapist in the State of New Hampshire (must be active at time of submission) - Required
- BLS (AHA) certification - Required
- 1-2 years of clinical experience preferred
- Compact license accepted where applicable; NH license must be active at submission
- Comfort supervising therapy assistants and mentoring students
Education
- Bachelor's Degree in Physical Therapy (accredited program) - Required
Knowledge & Skills
- Skilled in patient evaluation, treatment planning, therapeutic interventions, and re-evaluation
- Strong clinical documentation and daily billing accuracy
- Patient education and interdisciplinary communication to support coordinated care
- Experience or interest in pulmonary and cardiac rehabilitation is a plus
- Ability to supervise therapy assistants and participate in clinical instruction or mentoring
- Familiarity with quality improvement and performance improvement initiatives
- Efficient scheduling and time management to maximize patient access and flow
