Roles & Responsibilities

• Provide compassionate, high-quality bedside care to patients at the Pocahontas Center
• Administer medications, treatments and document interventions accurately
• Monitor and assess patient conditions, escalating changes to the care team promptly
• Collaborate with nurses, providers and support staff to deliver coordinated care
• Maintain clear, timely documentation in the patient record and support continuity of care
• Support patient and family education with empathy and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) license (LPN required; Registered Nurse applicants may be considered)
• Valid license to practice in West Virginia or ability to obtain state licensure
• Comfortable working 7:00 PM - 7:30 AM night shifts and providing hands-on patient care
• Strong communication skills, clinical judgment and ability to work as part of a team

Education

• Completion of an accredited practical nursing program (LPN) or equivalent

Knowledge & Skills

• Solid fundamentals in nursing assessments and bedside care
• Medication administration and documentation best practices
• Ability to perform patient monitoring and recognize changes in condition
• Team-oriented mindset and ability to follow care plans and physician orders
• Basic familiarity with electronic charting and clinical documentation
