Roles & Responsibilities

• Assess patients and develop individualized physical therapy treatment plans
• Deliver hands-on therapy, therapeutic exercise, and modalities to help patients regain function
• Monitor and document patient progress thoroughly and adjust care plans as needed
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Educate patients and their families on home programs, safety, and long-term mobility strategies
• Contribute to a positive, patient-centered culture by demonstrating professionalism and empathy

Qualifications

• Licensed Physical Therapist (PT) in New Hampshire or eligible for licensure
• Doctor of Physical Therapy (DPT) preferred; related degree acceptable
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a rehab/contract setting with varied patient populations
• Excellent communication skills and the ability to collaborate with an interdisciplinary team
• Professional, reliable, and committed to high-quality patient care

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient assessment, treatment planning, and functional mobility training
• Proficient with therapeutic exercise, manual therapy, gait training, and ADL retraining
• Experience with common outpatient/inpatient rehabilitation diagnoses and interventions
• Strong documentation skills and familiarity with electronic medical records
• Good clinical reasoning, time management, and patient education abilities
