Roles & Responsibilities

• Assess patients to determine functional status and establish measurable goals
• Develop and implement individualized treatment plans based on best practices
• Provide hands-on skilled therapy and progress timely toward goals
• Document patient progress accurately in the medical record
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Educate patients and families on home programs, safety, and continued recovery

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible to obtain state licensure
• Current Basic Life Support (BLS) certification preferred
• Experience working in a Skilled Nursing Facility is a plus
• Comfortable with electronic medical records and documentation standards
• Strong communication and teamwork skills

Education

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy or equivalent

Knowledge & Skills

• Thorough knowledge of physical therapy evaluation and intervention techniques
• Skilled in mobility training, gait training, transfers, and therapeutic exercise
• Ability to perform safe patient handling and fall-risk interventions
• Good clinical reasoning and goal-driven treatment planning
• Strong documentation skills and familiarity with regulatory compliance
