Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs using evidence-based evaluations
- Develop individualized treatment plans that restore independence and improve quality of life
- Deliver hands-on therapy and therapeutic exercise programs with compassion and clinical expertise
- Monitor patient progress and adjust plans to achieve safe, measurable outcomes
- Collaborate with interdisciplinary team members — nurses, OT, SLP, and physicians — to coordinate care
- Educate patients and families on home programs, safety strategies, and discharge planning
- Document progress clearly and timely in the facility’s medical record system
Qualifications
- Licensed Physical Therapist in Pennsylvania or eligible to obtain state licensure
- Current BLS/CPR certification (or willing to obtain prior to start)
- Comfort working in skilled nursing / rehab settings; prior experience in inpatient rehab or SNF preferred
- Strong clinical reasoning, patient-centered communication, and documentation skills
- Ability to work collaboratively within an interdisciplinary team and adapt to a fast-paced environment
Education
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (or equivalent professional degree)
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, transfers, and balance interventions
- Familiarity with mobility aids, assistive devices, and safe patient handling
- Competent with documentation and electronic health record systems
- Good patient and family education techniques, with a focus on functional outcomes
- Strong time management, prioritization, and teamwork abilities
