Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with facility policies
• Administer medications and treatments safely and accurately within your scope of practice
• Assess resident condition, monitor vital signs, and promptly report changes to the care team
• Document resident care and progress clearly and timely in the electronic health record
• Support individualized care plans and collaborate with RNs, physicians, and therapists
• Educate residents and families about care plans, medication, and safety tips
• Promote a safe, clean environment and adhere to infection control and regulatory standards

Qualifications

• Current and active LPN license (required) — Pennsylvania licensure preferred
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfortable working evening shifts and collaborating with an interdisciplinary team
• Strong communication skills and a patient-centered approach
• Previous long-term care or acute care experience preferred but new grads with strong clinical skills are welcome to apply

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Familiarity with common nursing assessments and vital sign interpretation
• Competence using electronic health records and completing accurate charting
• Good clinical judgment, prioritization, and time-management skills
• Ability to provide compassionate care and effective patient/family education
• Team-oriented mindset and strong collaboration skills
