Roles & Responsibilities
- Provide compassionate, hands-on care to residents, assisting with activities of daily living
- Observe and report changes in patient condition to the nursing team promptly
- Perform vital signs, basic clinical tasks, and accurate documentation with attention to detail
- Support resident mobility and fall-prevention efforts while promoting independence
- Assist with meal service, feeding, and maintaining a safe, clean care environment
- Collaborate with interdisciplinary team members to deliver consistent, person-centered care
Qualifications
- Active LNA / Licensed Nursing Assistant certification (VT or NH license preferred or ability to obtain)
- Current CPR certification (or willingness to obtain prior to start)
- Up-to-date TB test and immunizations as required by facility
- Background check and drug screen clearance as required
- Previous experience as an LNA or similar patient care role is preferred
- Reliable, compassionate, and able to work independently in a fast-paced setting
Education
- High school diploma or GED required
- Completion of LNA/CNA program and current LNA certification
Knowledge & Skills
- Skilled in assisting with ADLs, transfers, and resident hygiene
- Strong observational skills for recognizing clinical changes and communicating concerns
- Comfortable with basic clinical documentation and charting
- Effective communication and teamwork across nursing and support staff
- Ability to prioritize tasks and manage time during busy evening shifts
- Commitment to resident dignity, safety, and person-centered care
