Roles & Responsibilities

• Conduct comprehensive home-based patient assessments using OASIS and EPIC
• Develop and implement individualized plans of care and home exercise programs
• Provide therapeutic interventions such as gait training, strength testing and pain management
• Document evaluations, progress notes and discharge planning accurately in EPIC and OASIS
• Manage a focused caseload (may visit up to 6 patients per day) while meeting productivity expectations
• Educate patients and caregivers to support safety, independence and continuity of care
• Coordinate with the interdisciplinary team and community resources to optimize outcomes
• Travel safely between patient homes while minimizing drive time and prioritizing patient care

Qualifications

• Active Physical Therapist license (pending licenses accepted — please call out if pending)
• Minimum 1 year of skilled home health experience required
• BCLS certification required
• OASIS experience is required; EPIC & Rover charting experience strongly preferred
• Reliable transportation and willingness to travel within an occasional ~60 mile radius during the workday
• Comfort managing adult and geriatric patient populations with diagnoses like post-stroke, Parkinson’s and ortho post-op
• Weekend coverage is not required for this role

Education

• Graduate of an accredited Physical Therapy program and eligibility for PT licensure (pending licenses accepted)

Knowledge & Skills

• Patient assessment and clinical reasoning for home health settings
• Plan of Care development, documentation and OASIS completion
• Home Exercise Program creation and patient/caregiver education
• Gait training, strength testing and pain assessment techniques
• Experience with EPIC (remote client) and Rover mobile charting
• Modalities knowledge: E-stim, TENS, Ultrasound, cryotherapy and moist heat
• Familiarity with billing/payor considerations: Medicare A/B, Medicaid and commercial billing
• Skills in wound care, orthopedics, geriatrics and neurologic rehabilitation
