Roles & Responsibilities

• Provide compassionate, patient-centered care on the rehab unit to help patients regain independence
• Administer medications and treatments safely and accurately within scope of practice
• Monitor and document patient status, changes, and progress in the medical record
• Collaborate with interdisciplinary team members to coordinate individualized care plans
• Assist with mobility, restorative nursing tasks, and activities of daily living to support rehabilitation goals
• Educate patients and families about care plans, medications, and discharge needs
• Actively participate in shift handoffs to ensure continuity and patient safety

Qualifications

• Current, valid LPN or RN license in good standing (applicable to the role you apply for)
• Clinical experience in a rehab, long-term care, or similar setting preferred
• Strong communication and teamwork skills to collaborate with an interdisciplinary team
• Willingness to work day shift hours and adapt to facility procedures
• Ability to complete credentialing and standard pre-employment requirements

Education

• Graduate of an accredited practical nursing program for LPN roles
• Graduate of an accredited ADN/BSN program for RN roles

Knowledge & Skills

• Solid knowledge of basic nursing assessments and vital sign monitoring
• Medication administration and safe documentation practices
• Familiarity with rehabilitation nursing principles and restorative care
• Comfortable with electronic medical records and accurate charting
• Good time management, prioritization, and patient advocacy skills
• Professional bedside manner and ability to teach patients and families
