Roles & Responsibilities
- Assess patients thoroughly — obtain histories, perform systems reviews, and use tests and measures to pinpoint functional deficits
- Develop individualized, patient-centered treatment plans with clear goals to reduce pain, restore function, and prevent disability
- Perform ongoing reassessments and adapt treatments based on patient response
- Initiate and manage discharge planning from the first visit through transition
- Supervise and delegate appropriately to ancillary staff and students in line with state law and facility policy
- Complete clear, timely documentation that meets department, accreditation, and payer requirements
- Communicate effectively with patients, families, caregivers, and the interdisciplinary team using AIDET to maximize care outcomes
- Provide patient, family, and caregiver education tailored to individual needs
- Participate in professional development and quality improvement activities to continually raise standards of care
- Maintain a safe care environment by following approved safety standards and facility protocols
Qualifications
- 2 years of Physical Therapy experience in a Long Term Care (LTC) setting — required
- Current Texas Physical Therapy license or Compact privilege — required
- Proven ability to perform patient evaluations, differential diagnosis, and develop treatment plans
- Experience supervising ancillary staff and students consistent with state regulations
- Strong documentation skills and familiarity with accreditation and insurance documentation expectations
- Commitment to professional growth, quality improvement, and patient-centered care
