Roles & Responsibilities

• Assess patients thoroughly — obtain histories, perform systems reviews, and use tests and measures to pinpoint functional deficits
• Develop individualized, patient-centered treatment plans with clear goals to reduce pain, restore function, and prevent disability
• Perform ongoing reassessments and adapt treatments based on patient response
• Initiate and manage discharge planning from the first visit through transition
• Supervise and delegate appropriately to ancillary staff and students in line with state law and facility policy
• Complete clear, timely documentation that meets department, accreditation, and payer requirements
• Communicate effectively with patients, families, caregivers, and the interdisciplinary team using AIDET to maximize care outcomes
• Provide patient, family, and caregiver education tailored to individual needs
• Participate in professional development and quality improvement activities to continually raise standards of care
• Maintain a safe care environment by following approved safety standards and facility protocols

Qualifications

• 2 years of Physical Therapy experience in a Long Term Care (LTC) setting — required
• Current Texas Physical Therapy license or Compact privilege — required
• Proven ability to perform patient evaluations, differential diagnosis, and develop treatment plans
• Experience supervising ancillary staff and students consistent with state regulations
• Strong documentation skills and familiarity with accreditation and insurance documentation expectations
• Commitment to professional growth, quality improvement, and patient-centered care
