Roles & Responsibilities

• Provide compassionate direct patient care in accordance with facility policies and the scope of practice
• Administer medications, document administration, and monitor therapeutic effects
• Perform ongoing patient assessments and promptly report any changes in condition
• Collaborate with nurses, therapists, and providers to create and execute individualized care plans
• Maintain accurate, timely charting in the electronic medical record
• Assist with procedures and basic wound care per facility protocols
• Adhere to infection control, safety standards, and regulatory requirements

Qualifications

• Active Licensed Practical Nurse (LPN) license for New Mexico or ability to obtain state licensure quickly
• Basic Life Support (BLS) certification required; ACLS or PALS as applicable is a plus
• Current immunizations and ability to pass background check and drug screen
• Comfortable working overnight shifts and functioning independently on shift
• Previous experience in acute care or rehabilitation settings is preferred

Education

• Graduate of an accredited practical nursing program (LPN). For RN candidates listed on this posting, an Associate or Bachelor of Science in Nursing is appropriate.

Knowledge & Skills

• Strong patient assessment and monitoring skills
• Medication administration and documentation best practices
• Familiarity with electronic medical records and clear charting
• Good clinical judgment and the ability to escalate concerns appropriately
• Excellent communication and teamwork skills
• Basic wound care and mobility assistance knowledge
