Roles & Responsibilities

• Provide compassionate bedside nursing care to assigned patients in the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately according to orders
• Monitor and document patient status, vital signs and clinical changes in a timely manner
• Collaborate with the interdisciplinary team to develop and follow patient care plans
• Assist with admissions, discharges and transfers while ensuring patient comfort and safety
• Educate patients and families on care plans, medications and discharge instructions

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) classification listed for this assignment
• Active, unrestricted nursing license appropriate for the role
• Comfortable working a 12+ hour day shift (6:00 AM - 6:30 PM)
• Strong communication skills and a team-first mindset

• Ability to prioritize tasks in a busy rehab/healthcare setting
