Roles & Responsibilities

• Provide compassionate, patient-centered care during overnight shifts
• Administer medications and treatments accurately and chart thoroughly
• Perform timely patient assessments and report changes to the care team
• Collaborate with nurses, providers and support staff to ensure safe handoffs
• Educate patients and families about care plans and discharge instructions
• Maintain a safe environment and adhere to infection control and facility policies
• Support admission, transfer and discharge processes to keep patient flow smooth

Qualifications

• Active LPN license in New Mexico or eligibility to obtain licensure quickly
• Previous LPN experience preferred; experience in acute care or rehab is a plus
• Current BLS (Basic Life Support) certification required
• Ability to work overnight 12+ hour shifts and function independently
• Strong communication skills and a patient-first attitude

Education

• Graduation from an accredited Practical Nursing program (LPN certificate/diploma)

Knowledge & Skills

• Patient assessment and vital sign monitoring
• Medication administration and documentation
• Wound care basics and assistance with activities of daily living
• Familiarity with electronic health record (EHR) charting
• Ability to prioritize, multitask and escalate concerns appropriately
• Team collaboration and clear communication with interdisciplinary staff
