Roles & Responsibilities
- Provide compassionate, skilled nursing care to adult patients in a rehab/acute setting
- Administer medications and treatments safely and document timely and accurately
- Perform thorough patient assessments and escalate changes in condition
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Support patient mobility, rehab goals, and discharge planning with a patient-centered focus
- Maintain infection control standards and uphold facility policies and procedures
- Mentor and support peers while contributing to a positive clinical culture
Qualifications
- Active Licensed Practical Nurse (LPN) license — New Mexico license preferred or eligible for endorsement
- Open to Registered Nurse (RN) candidates if applicable to facility needs (hold RN license if applying as RN)
- Current Basic Life Support (BLS) certification or willingness to obtain promptly
- Recent clinical experience in medical-surgical, rehabilitation, or long-term care settings is a plus
- Strong communication skills, clinical judgment, and teamwork mindset
Education
- Graduate of an accredited LPN program; for RN applicants, graduation from an accredited associate or bachelor nursing program
Knowledge & Skills
- Patient assessment and monitoring with keen attention to changes in condition
- Medication administration and accurate documentation practices
- Wound care, IV experience (if applicable) and post-operative/rehab support
- Electronic health record (EHR) charting and care coordination
- Ability to prioritize tasks in a fast-paced clinical environment
- Professionalism, empathy, and effective interdisciplinary communication
