Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and treatments safely and accurately
- Monitor patient status, record observations, and escalate changes to the care team
- Collaborate with RNs, physicians, and multidisciplinary staff to implement care plans
- Support patient education and discharge planning to promote positive outcomes
- Maintain accurate, timely documentation and follow facility policies and procedures
Qualifications
- Current, unrestricted LPN license in New Mexico (required)
- Basic Life Support (BLS) certification required; additional certifications a plus
- Current immunizations and TB screening per facility policy
- Comfortable working in acute or rehab settings and collaborating with a team
- Professional attitude, strong communication skills, and a patient-first mindset
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care, IV site observation, and dressing changes
- Patient assessment skills and vital sign interpretation
- Familiarity with electronic medical records and charting
- Effective communication with patients, families, and interdisciplinary teams
