Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and treatments safely and accurately
• Monitor patient status, record observations, and escalate changes to the care team
• Collaborate with RNs, physicians, and multidisciplinary staff to implement care plans
• Support patient education and discharge planning to promote positive outcomes
• Maintain accurate, timely documentation and follow facility policies and procedures

Qualifications

• Current, unrestricted LPN license in New Mexico (required)
• Basic Life Support (BLS) certification required; additional certifications a plus
• Current immunizations and TB screening per facility policy
• Comfortable working in acute or rehab settings and collaborating with a team
• Professional attitude, strong communication skills, and a patient-first mindset

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care, IV site observation, and dressing changes
• Patient assessment skills and vital sign interpretation
• Familiarity with electronic medical records and charting
• Effective communication with patients, families, and interdisciplinary teams
