Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to rehab patients
• Administer medications and treatments accurately and on schedule
• Perform thorough patient assessments and communicate changes to the care team
• Collaborate with therapists and physicians to support individualized rehab plans
• Document clinical observations and interventions clearly in the medical record
• Support safe patient transfers, mobility programs, and fall-prevention efforts
• Educate patients and families on care plans, medications, and discharge goals

Qualifications

• Current, active LPN or RN license (must be valid in New Mexico)
• Minimum clinical experience in medical-surgical or rehabilitation settings preferred
• Basic Life Support (BLS) certification — obtain or maintain as required by facility
• Strong clinical assessment skills and comfort with medication administration
• Dependable, professional attitude and ability to work independently on night shift

Education

• LPN: Graduate of an accredited practical nursing program (or equivalent)
• RN: Associate or Bachelor of Science in Nursing (ADN/BSN) from an accredited program

Knowledge & Skills

• Skilled in medication administration, wound care, and patient assessments
• Experience with rehab-focused care: mobility support, ADL assistance, and fall prevention
• Proficient documentation skills and basic EMR familiarity
• Effective communicator with patients, families, and interdisciplinary teams
• Critical thinking, time management, and the ability to prioritize in a busy setting
