Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to adults in the Uptown Health/Rehab area
• Administer medications and treatments accurately and document per facility policy
• Perform timely assessments, monitor patient status and escalate changes to the care team
• Collaborate with interdisciplinary staff to develop and implement individualized care plans
• Maintain clear, concise charting in the electronic medical record and support continuity of care
• Support discharge planning and patient education to promote positive outcomes

Qualifications

• Active LPN or RN license in good standing (appropriate to the role you are applying for)
• Current BLS (Basic Life Support) certification required
• Clinical experience in a rehab or acute care setting preferred
• Strong communication and teamwork skills, with a professional, patient-first attitude
• Ability to work the listed shift hours and adapt to unit needs for a travel assignment

Education

• Licensed Practical Nurse: completion of an accredited LPN/LVN program
• Registered Nurse (if applicable): ADN or BSN from an accredited nursing program

Knowledge & Skills

• Solid medication administration and documentation practices
• Basic nursing assessments and vital sign interpretation
• Familiarity with electronic medical records and charting workflows
• Infection control, wound care fundamentals and safe patient handling
• Prioritization and time management during busy 12-hour day shifts
