Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the Uptown Health/Rehab area
- Administer medications and treatments accurately and document interventions
- Monitor patients' conditions, report changes, and collaborate with the care team
- Perform skilled nursing assessments and develop patient-centered care plans
- Support rehabilitation goals by assisting with ADLs and mobility as appropriate
- Communicate clearly with patients, families, and interdisciplinary staff
- Maintain accurate, timely documentation in the facility charting system
- Adhere to infection control, safety protocols, and facility policies
Qualifications
- Active LPN license in New Mexico (RN license also accepted — LPN or RN candidates welcome)
- Current BLS (Basic Life Support) certification; additional ACLS/PALS if required by facility
- Valid clinical experience in long-term care, rehab, or similar settings preferred
- Reliable, patient-focused attitude with strong communication and teamwork skills
- Ability to work the specified shift schedule and adapt to unit needs
Education
- Completion of an accredited practical nursing program (LPN)
- Associate's or diploma for RN candidates (if applying as RN)
Knowledge & Skills
- Skilled in medication administration and accurate charting
- Comfortable with wound care, IV basics, and mobility assistance
- Good clinical assessment and triage judgment
- Strong interpersonal skills for patient/family education and team communication
- Familiarity with rehab nursing practices and post-acute care workflows
