Roles & Responsibilities
- Provide compassionate, direct patient care to adults in the Uptown Health/Rehab area
- Administer medications and treatments accurately and safely
- Monitor and document patient status, responses, and progress in the chart
- Collaborate with physicians, therapists, and the interdisciplinary team to deliver coordinated care
- Educate patients and families about care plans, medications, and follow-up
- Maintain a safe, clean environment and follow infection control policies
- Support admissions, transfers, and discharges with professionalism and empathy
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license (New Mexico) — LPN or RN acceptable as listed
- Clinical experience in acute or rehab settings is a plus
- Facility-mandated certifications (facility may require BLS or others) as applicable
- Excellent communication, assessment, and teamwork skills
- Ability to work the posted day shift schedule and adapt to facility policies
Education
- Graduation from an accredited LPN or RN nursing program (as appropriate for the role)
Knowledge & Skills
- Strong patient assessment and clinical judgment
- Medication administration and documentation accuracy
- Familiarity with common rehab and inpatient nursing tasks
- Comfortable with electronic charting and documentation
- Good time management, prioritization, and teamwork
