Roles & Responsibilities

• Provide compassionate, direct patient care to adults in the Uptown Health/Rehab area
• Administer medications and treatments accurately and safely
• Monitor and document patient status, responses, and progress in the chart
• Collaborate with physicians, therapists, and the interdisciplinary team to deliver coordinated care
• Educate patients and families about care plans, medications, and follow-up
• Maintain a safe, clean environment and follow infection control policies
• Support admissions, transfers, and discharges with professionalism and empathy

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license (New Mexico) — LPN or RN acceptable as listed
• Clinical experience in acute or rehab settings is a plus
• Facility-mandated certifications (facility may require BLS or others) as applicable
• Excellent communication, assessment, and teamwork skills
• Ability to work the posted day shift schedule and adapt to facility policies

Education

• Graduation from an accredited LPN or RN nursing program (as appropriate for the role)

Knowledge & Skills

• Strong patient assessment and clinical judgment
• Medication administration and documentation accuracy
• Familiarity with common rehab and inpatient nursing tasks
• Comfortable with electronic charting and documentation
• Good time management, prioritization, and teamwork
