Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents
• Administer medications and treatments accurately and safely
• Monitor and document patient vital signs, symptoms, and changes in condition
• Collaborate with the interdisciplinary team to coordinate effective care plans
• Educate patients and families on condition management and discharge instructions
• Respond promptly to emergencies and escalate concerns as needed
• Maintain accurate, timely clinical documentation in the facility EMR
• Serve as a reliable, professional presence that supports patient dignity and comfort

Qualifications

• Current and unrestricted LPN license in New Mexico (or compact/eligible per state rules)
• BLS (Basic Life Support) certification required; ACLS preferred where applicable
• Previous experience in long-term care, rehab, or acute settings is a plus
• Ability to work night shifts and adapt to travel assignment schedules
• Up-to-date immunizations and ability to pass background and drug screening

Education

• LPN: Graduate of an accredited practical/vocational nursing program
• RN candidates: Associate Degree in Nursing (ADN) or BSN preferred

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with wound care, IV monitoring, and common nursing procedures
• Proficient with electronic charting and documentation best practices
• Excellent communication and teamwork skills with patients, families, and staff
• Good time management, critical thinking, and ability to prioritize
• Commitment to patient safety, infection control, and regulatory compliance
