Roles & Responsibilities

• Provide compassionate, safe nursing care to patients in the Uptown health and rehab area
• Administer medications and treatments accurately and document thoroughly
• Perform patient assessments and monitor changes in condition, escalating when appropriate
• Support and assist with rehabilitation activities and individualized care plans
• Collaborate with the interdisciplinary team to coordinate excellent patient care
• Maintain clear, timely EMR documentation and protect patient confidentiality
• Educate patients and families about care, discharge plans, and self-care strategies

Qualifications

• Current LPN license (ability to hold or obtain New Mexico licensure required)
• Basic Life Support (BLS) certification required
• Previous experience in skilled nursing, rehab, or acute care preferred
• Strong communication, teamwork, and clinical judgement skills
• Ability to pass background check and drug screening per facility policy

Education

• Graduate of an accredited Practical Nursing program and active LPN licensure

Knowledge & Skills

• Medication administration and IV/shot knowledge appropriate to LPN scope
• Basic wound care and dressing change experience
• Patient assessment and vital sign monitoring
• Comfortable with EMR documentation and charting
• Infection prevention practices and attention to safety
• Rehab support skills and working with therapy teams
• Time management and prioritization during busy shifts
