Roles & Responsibilities

• Provide compassionate, safe nursing care to patients while following facility policies and protocols
• Perform thorough assessments and document patient status, changes and interventions accurately
• Administer medications and treatments safely and on schedule, with attention to detail
• Collaborate with the interdisciplinary care team to develop and implement patient-centered plans
• Educate patients and families about care plans, discharge instructions and wellness goals
• Respond to patient needs and emergencies calmly and efficiently, prioritizing safety
• Support continuity of care by communicating clear handoffs and thorough charting

Qualifications

• Current, active LPN or RN license in the state of practice (LPN or RN acceptable)
• Basic Life Support (BLS) certification required or willingness to obtain promptly
• Clinical experience in acute care or rehabilitation settings preferred
• Ability to pass facility onboarding, background checks and required health screenings
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited practical nursing program for LPN or an accredited nursing program for RN
• Additional certifications or coursework in acute care or rehab nursing a plus

Knowledge & Skills

• Patient assessment and monitoring with attention to subtle changes
• Medication administration and accurate documentation
• Charting in electronic medical records and clear clinical handoffs
• Prioritization and time management during busy 12-hour shifts
• Comfort working with diverse patient populations in a fast-paced setting
