Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the Uptown Health/Rehab area
- Administer medications and treatments safely and accurately
- Monitor patient status and report changes promptly to the care team
- Collaborate with RNs, therapists, and physicians to create and follow individualized care plans
- Perform thorough documentation in the electronic medical record to ensure continuity of care
- Assist with admissions, discharges, and transfers with attention to patient comfort and safety
- Support rehabilitation activities and encourage patient independence
Qualifications
- Active Licensed Practical Nurse (LPN) license (New Mexico) — candidates with an active RN license are also welcome
- Basic Life Support (BLS) certification — or willingness to obtain upon hire
- Clinical experience in long-term care, rehab, or acute settings preferred
- Strong communication and teamwork skills; dependable and patient-focused
- Ability to pass facility background checks and meet credentialing requirements
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or equivalent required
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Comfortable with patient assessments, vital signs, and wound care
- Familiarity with electronic medical records and accurate charting
- Knowledge of infection control best practices
- Good clinical judgment, time management, and prioritization abilities
- Strong interpersonal skills for working with patients, families, and interdisciplinary teams
