Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately
• Monitor patient status and report changes promptly to the care team
• Collaborate with RNs, therapists, and physicians to create and follow individualized care plans
• Perform thorough documentation in the electronic medical record to ensure continuity of care
• Assist with admissions, discharges, and transfers with attention to patient comfort and safety
• Support rehabilitation activities and encourage patient independence

Qualifications

• Active Licensed Practical Nurse (LPN) license (New Mexico) — candidates with an active RN license are also welcome
• Basic Life Support (BLS) certification — or willingness to obtain upon hire
• Clinical experience in long-term care, rehab, or acute settings preferred
• Strong communication and teamwork skills; dependable and patient-focused
• Ability to pass facility background checks and meet credentialing requirements

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or equivalent required

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable with patient assessments, vital signs, and wound care
• Familiarity with electronic medical records and accurate charting
• Knowledge of infection control best practices
• Good clinical judgment, time management, and prioritization abilities
• Strong interpersonal skills for working with patients, families, and interdisciplinary teams
