Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in a rehab/health setting
• Administer medications and treatments safely and according to facility policy
• Perform accurate patient assessments and communicate changes to the care team
• Document patient care clearly and timely in the medical record
• Collaborate with nurses, therapists and physicians to deliver coordinated care plans
• Support patient mobility and recovery goals with dignity and encouragement
• Maintain infection control and safety standards to protect patients and staff

Qualifications

• Active LPN license in New Mexico or eligibility to obtain state licensure promptly
• Previous LPN clinical experience preferred, especially in rehab, acute care or med-surg
• Basic Life Support (BLS) certification required or willingness to obtain
• Strong communication skills and a teamwork mindset
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduate of an accredited Licensed Practical Nurse program
• High school diploma or GED required

Knowledge & Skills

• Solid patient assessment and monitoring skills
• Medication administration and documentation accuracy
• Comfortable working in a fast-paced clinical environment
• Good charting and electronic medical record familiarity
• Ability to prioritize tasks and advocate for patient needs
• Team collaboration and effective communication with interdisciplinary staff
