Roles & Responsibilities

• Provide high-quality, compassionate nursing care to patients in the rehab/health center
• Perform thorough patient assessments and communicate changes promptly
• Administer medications safely and document treatments accurately
• Collaborate with therapists, physicians and support staff to create and follow individualized care plans
• Support patient mobility, safety and rehabilitation goals during the shift
• Maintain clear, timely charting in the electronic medical record
• Participate in shift handoffs and communicate critical information to incoming staff

Qualifications

• Active Registered Nurse or Licensed Practical Nurse license in New Mexico required
• BLS (Basic Life Support) certification required; additional certifications welcomed
• Experience in rehab, long-term care or acute care settings preferred
• Strong assessment, medication administration and documentation skills
• Professional, patient-centered attitude and good teamwork skills

Education

• Graduate of an accredited nursing program: ADN, ASN or BSN for RNs; Practical Nursing diploma for LPNs

Knowledge & Skills

• Solid clinical assessment and patient observation skills
• Medication administration and safe documentation practices
• Familiarity with electronic medical record systems
• Ability to work independently overnight and escalate concerns appropriately
• Good communication skills with patients, families and care teams
