Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in the Uptown Health/Rehab area
• Assess, monitor and document patient status, recognizing changes and escalating when needed
• Administer medications and treatments accurately and safely according to policies
• Collaborate with interdisciplinary teams to create and execute individualized care plans
• Support rehabilitation activities and assist patients with restorative goals
• Educate patients and families on care plans, medications and safe transitions

Qualifications

• Active RN or LPN license in good standing (New Mexico licensure preferred or compact status)
• Current BLS certification required; ACLS or specialty certifications encouraged where applicable
• Prior clinical experience in med-surg, rehab or long-term care is preferred
• Strong communication skills and ability to work effectively with a multidisciplinary team
• Ability to pass facility onboarding and credentialing for travel assignments

Education

• LPN: Vocational/Practical Nursing diploma or equivalent
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Solid patient assessment and clinical judgment
• Medication administration and documentation accuracy
• Experience with rehabilitation care principles and restorative nursing
• Comfortable using electronic health record systems for charting
• Ability to prioritize, multitask and maintain a calm presence in busy settings
