Roles & Responsibilities

• Provide compassionate bedside nursing care to patients in the Uptown Health/Rehab area during overnight hours
• Administer medications and monitor patient responses accurately and safely
• Perform routine assessments and report changes in condition to the care team
• Document care and interventions clearly in the facility charting system
• Support rehabilitation goals by assisting with treatments, mobility, and patient education
• Collaborate with RNs, therapists, and interdisciplinary staff to deliver coordinated care
• Participate in infection control practices and maintain a safe patient environment
• Respond promptly and effectively to patient needs and overnight emergencies

Qualifications

• Active Licensed Practical Nurse (LPN) credential (or active RN credential if applying as an RN) — state licensure required
• Current Basic Life Support (BLS) certification (or ability to obtain promptly)
• Comfortable working overnight shifts in a rehab/long-term care setting
• Professional communication skills and a team-first attitude
• Ability to pass facility onboarding requirements and background checks

Education

• Graduate of an accredited Practical Nursing program for LPN candidates (or appropriate nursing degree for RN candidates)

Knowledge & Skills

• Strong medication administration and documentation skills
• Experience or comfort working in a rehabilitation/long-term care environment
• Basic assessment, wound care, and patient monitoring skills
• Familiarity with electronic health records and charting
• Good time management, prioritization, and teamwork abilities
