Roles & Responsibilities

• Provide hands-on, compassionate nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Monitor patient condition and report changes promptly to the care team
• Document assessments, interventions, and outcomes in the electronic record
• Collaborate with interdisciplinary team members to create and follow care plans
• Educate patients and families about care plans, medications, and safety
• Assist with admissions, discharges, and transfers with professionalism
• Respond to emergencies calmly and according to facility protocols

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
• Current Basic Life Support (BLS) certification required; ACLS preferred for RNs
• Recent clinical experience in rehabilitation, long-term care, or acute care is a plus
• Strong clinical assessment, medication administration, and documentation skills
• Demonstrated ability to work independently on overnight shifts and as part of a team
• Up-to-date immunizations and ability to complete facility onboarding requirements

Education

• Graduate of an accredited LPN or RN nursing program
• Additional specialty certifications welcomed but not required

Knowledge & Skills

• Solid knowledge of nursing fundamentals, medication administration, and wound care
• Comfortable using electronic medical records and documenting care accurately
• Excellent communication and patient education skills
• Ability to prioritize tasks in a fast-paced clinical environment
• Team-oriented mindset with strong collaboration skills
• Good critical thinking and clinical decision-making abilities
• Familiarity with infection control and safety best practices
• Compassionate bedside manner and respect for patient dignity
