Roles & Responsibilities

• Provide safe, compassionate direct patient care to adults in the Uptown Health/Rehab area
• Perform thorough assessments and communicate changes in patient status promptly
• Administer medications and treatments accurately and document per facility policy
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Assist with rehabilitation-focused activities and support patient independence
• Maintain clear, timely documentation and participate in shift handoffs

Qualifications

• Current and active LPN or RN license (must meet state requirements)
• Basic Life Support (BLS) certification required; other certifications (ACLS, PALS) a plus
• Clinical experience in nursing or rehab settings preferred
• Strong communication skills and an ability to work collaboratively
• Ability to pass required background checks and credentialing

Education

• Graduation from an accredited LPN program or an accredited nursing program for RNs (ADN or BSN preferred for RNs)

Knowledge & Skills

• Patient assessment, vital signs, and documentation best practices
• Medication administration and safe handling procedures
• Familiarity with rehab nursing principles and mobility assistance
• Infection control, wound care basics, and safety precautions
• Time management, clinical judgment, and teamwork skills
