Roles & Responsibilities

• Provide compassionate direct patient care including assessments, medication administration, and wound care
• Collaborate with interdisciplinary team members to create and update patient care plans
• Communicate clearly and professionally with patients, families, and staff to ensure safe, coordinated care
• Document patient status and care activities accurately in the electronic medical record
• Monitor and respond to changes in patient condition, escalating concerns to providers as needed
• Support safe patient transitions and discharge planning with a focus on positive outcomes

Qualifications

• Active LPN or RN license in New Mexico (or compact/multi-state where applicable)
• Current Basic Life Support (BLS) certification required; ACLS preferred depending on unit
• Clinical experience in acute care or rehab settings preferred
• Strong assessment, critical thinking, and patient advocacy skills
• Professional communication and teamwork mindset

Education

• Completion of an accredited LPN or RN program (Diploma, ADN, or Practical Nursing certificate as appropriate)
• Ongoing continuing education encouraged

Knowledge & Skills

• Patient assessment and monitoring
• Medication administration and documentation
• IV therapy and wound care competency where applicable
• Familiarity with electronic medical records and charting
• Infection control practices and patient safety standards
• Ability to prioritize and manage a fast-paced patient load
