Roles & Responsibilities

• Provide high-quality, compassionate nursing care to adults and/or seniors in the facility
• Administer medications and treatments accurately and document thoroughly
• Perform timely patient assessments and communicate changes to the interdisciplinary team
• Support safe transitions of care, including admissions, discharges and transfers
• Collaborate with therapists and physicians to implement individualized care plans
• Maintain accurate charting in the electronic health record and ensure regulatory compliance
• Mentor and support peers, fostering a positive, team-oriented environment

Qualifications

• Active LPN license in good standing OR active RN license in good standing
• Basic Life Support (BLS) certification required; ACLS or PALS if applicable to unit
• Current immunizations and ability to pass facility background checks and drug screen
• Clinical experience in acute care, rehabilitation, or long-term care preferred
• Strong communication skills and ability to work effectively with a multidisciplinary team

Education

• LPN: Graduate of an accredited practical nursing program (state licensure required)
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) (state licensure required)

Knowledge & Skills

• Skilled in patient assessment, medication administration, and care planning
• Comfortable with wound care, monitoring vital signs, and post-procedure nursing tasks
• Familiarity with electronic health records and clear clinical documentation
• Good triage instincts, safety-first mindset, and infection control practices
• Team player with strong time-management and prioritization abilities
