Roles & Responsibilities
- Provide compassionate, patient-centered nursing care in a rehabilitation/acute care setting
- Administer medications and treatments accurately and document per facility policy
- Monitor patient condition, record vital signs, and report changes to the care team
- Collaborate with RNs, therapists, and physicians to create and follow individualized care plans
- Assist with wound care, ADLs, transfers, and mobility to support patient recovery
- Educate patients and families about care plans, medications, and discharge instructions
- Maintain accurate, timely documentation in the electronic medical record
- Promote a safe environment by following infection control and safety protocols
Qualifications
- Current LPN license (Licensed Practical Nurse) in New Mexico or compact/state-appropriate licensing
- Basic Life Support (BLS) certification or willingness to obtain prior to start
- Current immunizations and health screening per facility policy
- Comfort working in a fast-paced rehabilitation/acute care environment
- Strong communication skills and a team-first attitude
- Previous clinical experience preferred; ready to jump in and make an impact
Education
- Graduate of an accredited practical nursing program (LPN) or equivalent nursing program
Knowledge & Skills
- Medication administration and basic nursing procedures
- Vital signs assessment and clinical monitoring
- Electronic medical record (EMR) documentation skills
- Infection control and patient safety practices
- Wound care and mobility/transfer assistance
- Patient and family education with clear, compassionate communication
- Ability to prioritize, multitask, and collaborate across disciplines
