Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in a rehabilitation/acute care setting
• Administer medications and treatments accurately and document per facility policy
• Monitor patient condition, record vital signs, and report changes to the care team
• Collaborate with RNs, therapists, and physicians to create and follow individualized care plans
• Assist with wound care, ADLs, transfers, and mobility to support patient recovery
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain accurate, timely documentation in the electronic medical record
• Promote a safe environment by following infection control and safety protocols

Qualifications

• Current LPN license (Licensed Practical Nurse) in New Mexico or compact/state-appropriate licensing
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Current immunizations and health screening per facility policy
• Comfort working in a fast-paced rehabilitation/acute care environment
• Strong communication skills and a team-first attitude
• Previous clinical experience preferred; ready to jump in and make an impact

Education

• Graduate of an accredited practical nursing program (LPN) or equivalent nursing program

Knowledge & Skills

• Medication administration and basic nursing procedures
• Vital signs assessment and clinical monitoring
• Electronic medical record (EMR) documentation skills
• Infection control and patient safety practices
• Wound care and mobility/transfer assistance
• Patient and family education with clear, compassionate communication
• Ability to prioritize, multitask, and collaborate across disciplines
