Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for assigned residents and patients
• Administer medications and treatments accurately and document per facility policy
• Assess patient condition, report changes to interdisciplinary team, and escalate concerns
• Coordinate with nursing staff and therapists to support care plans and discharge goals
• Maintain accurate and timely clinical documentation in the patient chart
• Support infection control practices and maintain a safe clinical environment
• Mentor and communicate clearly with peers, patients, and families to improve outcomes

Qualifications

• Active LPN or RN license in good standing (LPN or RN accepted)
• Current Basic Life Support (BLS) certification required
• Immunizations and health screenings as required by facility policy
• Previous experience in a long-term care, rehabilitation, or acute care setting preferred
• Strong clinical assessment, medication administration, and documentation skills
• Available to work overnight shifts and adapt to travel assignment requirements

Education

• LPN: Vocational nursing diploma or equivalent
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) as applicable

Knowledge & Skills

• Solid knowledge of medication administration, wound care, and clinical assessments
• Familiarity with electronic medical records and charting best practices
• Ability to prioritize tasks in a fast-paced overnight environment
• Excellent communication and teamwork skills with patients and multidisciplinary teams
• Understanding of infection control, safety protocols, and regulatory compliance
