Roles & Responsibilities

• Provide high-quality patient care and monitor patient status throughout the overnight shift
• Administer medications and treatments as ordered and document thoroughly
• Collaborate with the care team to support smooth handoffs and coordinated care
• Perform patient assessments, reporting changes promptly to the appropriate clinician
• Support rehabilitative and restorative care in the Uptown Health/Rehab area
• Maintain accurate, timely clinical documentation and follow facility policies

Qualifications

• Current LPN license (or RN license if applying as an RN) appropriate for role
• Basic Life Support (BLS) certification preferred or available upon hire
• Comfortable with overnight shifts and caring for patients in a rehab/long-term care setting
• Professional, compassionate approach with strong communication skills
• Ability to pass facility onboarding and background checks

Education

• Graduate of an accredited practical nursing program or equivalent

Knowledge & Skills

• Solid practical nursing skills including medication administration and wound care
• Familiarity with documentation best practices and electronic medical records
• Patient assessment and monitoring skills for acute and post-acute populations
• Team-oriented mindset with the ability to work independently overnight
• Time management and prioritization during busy shift periods
