Roles & Responsibilities
- Provide skilled physical therapy evaluations and hands-on treatments to SNF residents
- Design individualized, goal-driven care plans that help patients regain mobility and independence
- Deliver therapeutic interventions for strength, balance, gait, and functional tasks
- Document concise, timely, and compliant progress notes and discharge summaries
- Collaborate with nursing and interdisciplinary team members to optimize patient outcomes
- Coach patients and families on safe transfers, home programs, and fall prevention strategies
- Participate in care conferences and contribute to measurable rehab goals
Qualifications
- PT or DPT credentials noted in the job skills (PT, DPT)
- Any experience level accepted — years of experience: any, including new graduates
- Comfort working in a Skilled Nursing Facility (SNF) practice setting
- Strong communication skills and a team-oriented mindset
- Ability to complete required documentation and meet facility policies
Education
- DPT preferred (Doctor of Physical Therapy)
